To live on campus, students must complete the Meningococcal Vaccination Certification form. The University Health History Questionnaire is a record of specific vaccinations required by the University prior to attendance.

- Complete the form, indicating one of the following options
- You have received the vaccination from your health care provider and your provider has signed and dated the form.
- You have received the vaccination and attached a copy of the immunization record.
- You are requesting an exception from the immunization requirement.
NOTICE: In order to comply with state law, student must submit the form before arriving on campus, or the Act instructs that students will be prohibited from residing on campus.
