Vaccination Certification Form
If this is your first time living on campus at Penn
State, you must submit a completed Meningococcal Vaccination
Certification form to Housing and Food Services before
or at check-in in order to receive a key. If
you are a returning student and already have a form
on file, you will not have to submit a new one. All
on-campus students must have a completed form on file
in order to live in the residence halls.

Vaccination Certification form should be mailed directly
to the Penn State Altoona Housing and Food Services
office.
The Health
and Wellness Center can also provide additional
information about the meningitis vaccine and the availability
of a vaccination clinic on campus.
Health
History Questionnaire
All students attending Penn State must complete the University Health History Questionnaire, which is a record of specific vaccinations required by the University to be completed prior to attendance. This questionnaire is separate from the Meningococcal Vaccination Certification form and does not meet the Pennsylvania law requirement for on campus residential status. Students living on campus must complete both forms, including students that have received the meningococcal vaccination from University Health Services or have already submitted the Health History Questionnaire online. The Meningococcal Vaccination Certification form must be sent directly to the campus Housing and Food Services office.
