Meningococcal Vaccination

Meningococcal Vaccination Certification Form
All Penn State students residing in on-campus housing, including all residence halls and apartments, are required by state law to complete a Meningococcal Vaccination Certification online form and file it with University Health Services.

The online form must be completed prior to check-in.

The state law requiring the vaccination is a part of the College and University Student Vaccination Act. Former Governor Mark Schweiker and the Pennsylvania State Legislature passed the act in June 2002.

If a student lives on campus, they are required by state law to complete and submit the Meningococcal Vaccination Certification form, indicating they have complied through one of the following options:

  • Received a meningococcal vaccination through a health care provider or University Health Services.
  • They are electing to choose an exemption which confirms they have made an informed decision to not receive the meningococcal vaccination.

The Meningococcal Vaccination Certification form is found at University Health Services by clicking on "Health Forms".

Health Services can also provide additional information about the meningitis vaccine and the availability of a vaccination clinic on campus.

Health History Questionnaire

All students attending Penn State must complete the University Health History Questionnaire, which is a record of specific vaccinations required by the University to be completed prior to attendance. This questionnaire is separate from the Meningococcal Vaccination Certification form and does not meet the Pennsylvania law requirement for on campus residential status. Students living on campus must complete both forms, including students that have received the meningococcal vaccination from University Health Services or have already submitted the Health History Questionnaire online. The Meningococcal Vaccination Certification form must be sent directly to the campus Housing and Food Services office.
