To live on campus, students must complete the Meningococcal Vaccination Certification form. In addition, University Health Services requires all students complete the University Health History Questionnaire, which is a record of specific vaccinations required by the University prior to attendance.

- You have received the vaccination.
- You are requesting an exemption from the immunization requirement.
NOTICE: In order to comply with state law, students must submit the form before arriving on campus, or the Act instructs that students will be prohibited from residing on campus.
